CSHBC recently updated all Certified Practice Certification Program & Application forms to a more user-friendly fillable PDF format. In addition, all application forms now include an extension section where the registrant applying for a Certified Practice Certification Program and Certified Practice supervisor indicate whether any objectives in the training program were extended, and the rationale for the extension.
To assist CSHBC in streamlining its review process for applications, a universal Practical Learning Log is embedded in all the updated Certified Practice Certification Program & Application forms. Logging the specific minimum observation and practical training requirements under supervision, as outlined in each certification program, is now required within every application submission.
