On December 23, 2022, Ms. Agnes Gradowski, RSLP, entered into an Undertaking and Consent Agreement consenting to an order for extraordinary action, pursuant to ss. 35(1) of the Health Professions Act. The College’s Inquiry Committee (the “Committee”) is investigating Ms. Gradowski’s practice regarding competency concerns.
Background
Ms. Gradowski is a registrant and speech language pathologist. In September 2022, the College received a report concerning Ms. Gradowski. The Inquiry Committee proceeded on its own motion to evaluate the grounds for concern, relating to Ms. Gradowski’s ability to practice competently in several roles.
Resolution
The Committee has ordered interim limits or conditions on Ms. Gradowski’s practice, during its investigation, with Ms. Gradowski’s consent. The limits or conditions require that, during the investigation and pending a hearing if any, Ms. Gradowski’s practice be supervised by another RSLP respecting dysphagia-related activities, and respecting classes of certified practice activity, specifically Certificates E (Flexible Endoscopic Evaluation of Voice & Swallowing), F (Flexible Endoscopic Evaluation of Swallowing), and I (Video Fluoroscopic Assessment & Management of Adult Swallowing Disorders).
The Committee believes that this interim action is sufficient to protect the public during its investigation.
