On October 5, 2021 Ms. Agnes Gradowski, RSLP, entered into an Undertaking and Consent Agreement, pursuant to ss. 33(6)(c) and 36(1) of the Health Professions Act. The College’s Inquiry Committee (the “Committee”) investigated Ms. Gradowski regarding competency concerns.
Background
Ms. Gradowski is a College registrant and speech language pathologist. In July 2020, the College received reports concerning Ms. Gradowski which the registrar delivered to the Inquiry Committee. Ms. Gradowski agreed to limits on her practice pending the completion of the College’s investigation. During its investigation, the Inquiry Committee evaluated the grounds for concern relating to Ms. Gradowski’s practice in dysphagia management.
Resolution
The Inquiry Committee resolved to dispose of matters pursuant to ss. 33(6)(c) and 36(1) of the HPA by requiring that Ms. Gradowski undertake or consent to certain practice conditions. The practice conditions included completion of five dysphagia assessments under the supervision of a supervisor approved by the College, as well as attending two practice consultations. Following completion of these requirements, the supervisor reported their findings to the Inquiry Committee, which resulted in additional recommendations to improve Ms. Gradowski’s practice.
The Inquiry Committee required Ms. Gradowski to indemnify the College for the supervision expenses. The Inquiry Committee believes this resolution is consistent with the College’s mandate to serve and protect the public.
