On December 8, 2022, Ms. Nicolette Waite, RSLP, entered into an Undertaking and Consent Agreement, pursuant to ss. 33(6)(c) and 36(1) of the Health Professions Act (HPA).
Ms. Waite is a registrant and a speech-language pathologist. In April 2022, the College received a complaint disclosing grounds of concern relating to Ms. Waite’s professionalism while declining to reschedule an appointment after several cancellations, and her maintaining the confidentiality of client information from her spouse.
The Inquiry Committee resolved to dispose of matters pursuant to ss. 33(6)(c) and 46(1) of the HPA by the Committee requesting, and the Respondent voluntarily providing, consents or undertakings for certain practice conditions. The conditions include
- an in-person practice assessment by a College assessor, relating to how Ms. Waite manages confidentiality, document client matters, and procedures for ending client relationships;
- an Assessor report to the Committee with recommendations;
- if the Assessor recommends supervision, supervision sessions shall not to exceed five sessions; and
- the Respondent’s undertaking to carry out the recommendations of the Assessor.
The Committee believes this resolution is sufficient to satisfy the College’s mandate to serve and protect the public.
