On October 3, 2023, Sherri Millham (the “Registrant”) entered into an Undertaking and Consent Agreement (the “Agreement”) following an investigation by the College’s Inquiry Committee (the “Committee”) into allegations that from July-August 2022, the Registrant failed to conduct an adequate Videofluroscopic Swallow Study (“VFSS”), made a clinically insupportable diagnosis based on the inadequate VFSS, failed to document important information on the VFSS report, and failed to provide follow-up care to the relevant patient following the VFSS.
Resolution
The Inquiry Committee resolved to dispose of the matter by requiring that the Registrant undertake or consent to certain practice limits and conditions. These include the following:
- The Registrant may only conduct activities within the scope of CSHBC Certified Practice Certificate I: Videofluoroscopic Assessment and Management of Swallowing Disorders – Adults under the supervision of a College-approved supervisor.
- The Registrant’s supervisor will report to the Committee on their findings from the supervision period.
- The Registrant may not supervise, train or otherwise assist other registrant in their working towards obtaining Certified Certificate I until they have successfully completed certain terms in the Agreement.
- The Registrant must successfully complete the College’s online Documentation & Records Management course, then complete at least one but up to three Practice Advice sessions with a College Professional Practice Advisor to review their understanding of this standard.
The Inquiry Committee believes this resolution is consistent with the College’s mandate to serve and protect the public.
